(KNSI) — Residents in Benton, Sherburne, and Stearns Counties will be randomly chosen to get a community health survey in the mail from CentraCare.
More than 65-hundred households in the tri-county area will get the survey, which is intended to gather information about health and wellness in local communities.
The survey will ask questions about access residents have to health care, nutrition, tobacco use, exercise, and mental health-related topics. The surveys will be mailed out this month, and the answers are completely confidential.
Residents are being asked to fill it out and send it back. The surveys are used better to understand area residents’ health status and needs and identify potential barriers to health care and healthy living.
