(KNSI) – A head-on crash near Lake Mille Lacs has claimed the lives of four people and left a fifth with life-threatening injuries.

According to the Minnesota State Patrol preliminary crash report, a 2004 Buick LeSabre and a 2005 Saab had their airbags deploy on impact when they collided on Highway 169, just north of Shakopee Road, around 7:00 p.m. on Wednesday near Onamia. Alcohol is listed as pending investigation for everyone involved in the crash.

The driver of the Buick LeSabre, 21-year-old Samantha Jean Ajax of Isle, was killed in the crash. It’s unknown if she was wearing her seatbelt. Two passengers, 41-year-old Christopher Andrew Boswell and 25-year-old Jessica Faye Mitchell, both of Onamia, were also in the car. Boswell was killed, and Mitchell sustained life-threatening injuries and was transported to St. Cloud Hospital. Neither was wearing a seatbelt.

The driver of the Saab 9-5, 53-year-old Michelle Marie Rock of Minneapolis, was fatally injured. A passenger, 51-year-old James Allen Ortley of Minneapolis, was also killed. Neither was wearing a seatbelt.

The investigation remains ongoing.
