(KNSI) – CentraCare is consolidating its family medicine services in Sartell with a move to a larger facility next week.
The health system will relocate to the CentraCare Sauk Crossing building at 2000 23rd Street South starting Monday, January 26th. The facility has housed the Eye Center since 2017 and has been renovated to add primary care services.
Primary care will move from the current CentraCare Sartell clinic, across the street, to Sauk Crossing.
CentraCare CEO Dr. Ken Holmen says the consolidation is meant to make it easier for patients to access care locally and support what he calls modern care models.
The first floor will include the Eye Center, imaging and lab services, and the School of Diagnostic Imaging. Family medicine, integrated behavioral health, and medication therapy management will be on the second floor.
